Are there any UTXO that can not be spent?

I have heard that there are UTXO that can not be spent.
From my understanding it is because ScriptPubKey is constructed in that way.

If it is correct, is there some real life examples on blockchain ?
What this done intentionally or by mistake ?
How many are there ?
